In an earlier issue of Cook’s Log,¹ George Burr described and illustrated this medal issued by the Birmingham Mint. 

Among other things, he wrote that the medal was minted in 1975, and does not appear in Klenman’s book.² 

I have found the medal illustrated in Klenman’s book with the reference number K86, and with an issue date of 1974. 

Burr also wrote that the medal was displayed in a red small hardback booklet containing a potted history of nine pages describing the achievements of the individual.
